We have a NOS FOMOCO choke thermostat control assembly, part number is C2AZ-9850-C. It is in great shape, you can still feel the spring tension when you turn the lever. It doesn't look like it has ever been bolted on. It might have some parts missing, I am not an old Ford parts expert. There is a clip and bracket glued to the cover.(see 5th picture) The box shows it's age with a missing flap and shelf wear.
