This OEM driver side HID headlight assembly came from a 2009 Lincoln MKS that was parted due to accident damage. The HID ballast is in proper working condition (see photos of it powered on). All of the mounting tabs are there but the top one has a crack and chip at the tip, this does not affect function (see last photo and it will be plastic welded to bolster it prior to shipping).
